Return Merchandise Authorization (RMA Form)
For prompt processing of return and item exchange, please complete and fax this
form to Dr. Battery
at 604-909-1828 or email to
rma@drbattery.com. Your RMA form will then be faxed or emailed back to you along with a RMA Number for your record. The RMA number will cancel if the product
is not returned within 15 days. The return item MUST be in its Original Packaging
and must be accompanied by a copy of the original invoice. The package must be prepaid,
sent by regular mail only and with the RMA number clearly marked on the shipping
label. (See: RMA Procedures)
RMA Procedures
STEP 1: REQUEST FOR RMA
Please download, complete and fax the RMA Form
to 604-909-1828 or email it to rma@drbattery.com.
To download the RMA Form, Click Here. You will
be advised within 48 hours whether the fault is covered under our warranty.
STEP 2: SHIP DEFECTIVE PRODUCT
Ship and insure the defective product(s) along with the RMA form (with RMA number)
and Copy of original invoice back to us in a protected packaging
within 7 days. (The RMA number will cancel if
the product is not returned within 15 days.) The package must be
prepaid, sent by Regular mail only and with the RMA number
clearly marked on the shipping label.

STEP 3: RECEIVE REPLY FOR RMA RESULT
